When routine neonatal circumcision/slitting of prepuce is not an individual or group customer benefit, claims for this procedure will be given individual consideration when the patient's clinical records document the medical necessity for the procedure. It will be necessary for the provider to submit medical records and/or additional documentation to determine coverage in this situation.
